Your Visit

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time you will save significant time at the time of your visit. If you have any questions when filling out these forms, please do the best you can, our staff will assist you with your questions on the day of your visit.

Your Visit

Before your first appointment, we will call you at home to explain some of our procedures. On your initial visit, you will be asked to complete a health history and information form. Forms are available above if you would like to complete them ahead of time and bring to your first visit. We will request your medical records from your prior doctors and hospitals, as well as a list of your current medications in order to formulate our best recommendations for you. All information given to us is strictly confidential and will not be released to anyone without your approval.

During your initial visit, your blood will be drawn to allow us to get baseline lab values for treatment. Though rare, it may be necessary to start your treatment immediately; however, in most cases, your treatment regimen will be scheduled to allow you to plan ahead. You will also receive an information packet that will include dietary guidelines and a comprehensive explanation about your chemotherapy or other medications, including side effects and what to expect. We will also tell you about our comprehensive services, cancer support groups in the area and more.

We will also review your prior medical records and test results. After a physical examination, we will outline a treatment plan and may order laboratory work. We would be happy to answer any questions at that time. If there is a need for chemotherapy and/or radiation therapy, one of our oncology nurses will meet with you individually to further explain our procedures and answer any of your questions.

We make every effort to see you at your scheduled time. However, our schedules are occasionally disrupted by emergencies. When that occurs, we do our best to inform you of any delay. Please understand and bear with us. Likewise, if you find you must cancel an appointment, please notify us as soon as possible.

We provide consultation, treatment, and follow-up care to patients referred to our specialty by the primary care physicians of the Treasure Coast and surrounding communities.
